The Prevention of disease or illness is the best method for maintaining our general health.  Therefore, we need to periodically check our general health conditioning.  It is essential to have a balanced diet, regular exercise, a good living environment and proper mental conditioning.

You can complete it online and submit it electronically; or if you prefer, you may print this page, complete it by hand, and fax or mail to the address below.  What follows is a self -check list that you can use in the event that you are not able to visit our office for a regular check up.  Please forward the completed questionnaire to our office so that we may review your results and respond appropriately to your current general health needs.  

General Health Conditioning remedies (in herbal form) should be taken every six months for about two weeks to insure that our body condition remains in good balance.
